Grant Purpose:
to improve health outcomes among infants born to low-income and/or high-risk mothers in Northeast and East Central Kansas through home visits. Within 36 hours of the baby’s release from the hospital, nurses will complete medical, environmental and social assessments, educate the mother on care of the infant and refer her to additional services and community resources that are available to her. This grant is intended to pay for staff salaries and travel.
