Grant Purpose:
for a project that aims to identify all Wyandotte County children younger than 3 who have disabilities or developmental delays. Program staff will help the families of these children identify a medical home, communicate with medical providers and locate resources including housing, transportation, employment and medical benefits. This grant is intended to pay for personnel, translators and travel associated with home visits.
