Healthy Communities Initiative
The Kansas Health Foundation is committed to improving the health of all Kansans, and one of our key focus areas is to promote healthy behaviors, including ensuring proper nutrition, increasing physical activity and decreasing tobacco use. On this page you will find information regarding the Foundation’s Healthy Communities Initiative, which will use a Request For Proposal (RFP) process to support communities as they create a healthier environment for their citizens. We invite you to please read the downloadable information contained on this page, which includes the actual RFP document, a Frequently Asked Questions document, the planning grant proposal templates and the review committee templates. We have also provided information about our electronic application process.

Proposal and Review Templates
For your convenience, we have included a downloadable template of the Healthy Communities Initiative Planning Grant Proposal containing the information you will be asked to complete on our electronic application form (described in more detail below). However, please keep in mind that the official submission of your information must be done through our online application system.
An independent review committee will evaluate all of the submitted proposals. We have provided a downloadable template of the review instructions the committee members will receive upon reviewing the proposals for the Healthy Communities Initiative.
